The following research survey is designed to provide the WV Center for Drug and Health Information with valuable feedback about the HTip project. It will allow us to gather information about peoples use of the Internet to obtain health-related information. This survey should take only a few minutes to complete.
Your feedback will be used to gather information to help the WV Center for Drug and Health Information (WV CDHI) improve and expand the HTip project. We will also use the responses obtained from this survey for professional research. This research will involve collecting information on Internet usage habits, types of information retrieved from the Internet, patient satisfaction with health care professionals providing quality health-related Internet sites, and patients willingness to discuss the health-related information with their physician. The sum of everyone's responses will be used for this information. This information will also help the WV CDHI expand this project by obtaining additional funding from other sources.
The answers to the survey questions will be delivered to us anonymously. We will not have access to any information that would identify the person completing this survey. In addition, all responses will be compiled together to gain information. No one individual response will be used solely when describing any information obtained from this project.
Participation in this survey is voluntary. You are not obligated to answer every question.
Your ability to use the HTip web site or be involved with the HTip project will in no way be affected if you choose not to complete this survey or if you choose not to answer certain questions on the survey.
